Who Can Apply
• Must meet the eligibility criteria in Section 2 of the guidelines for registration.
• Must meet the eligibility requirements in Section 3 of the guidelines for payment.
• Practices, providers and patients participating in MyMedicare GPACI must satisfy all eligibility requirements concurrently within an incentive period.
• Must register for MyMedicare and opt in to GPACI.
• Must address all eligibility criteria in the application/registration process.
• Cannot receive funding from another government source for the same purpose.
Eligibility is set out in Section 2 (Eligibility Criteria to register) and Section 3 (Eligibility requirements for payment) of the guidelines. Practices, providers and patients participating in the MyMedicare GPACI must meet all eligibility requirements concurrently within an incentive period for the practice or practitioner to be eligible for payment. Applicants must register for MyMedicare and opt in to GPACI, satisfy all eligibility criteria, and cannot receive funding from another government source for the same purpose.
