• Be a non-government organisation • Be a not-for-profit organisation • Ensure the activity is delivered within the Newcastle and Lake Macquarie region • Be a current contracted provider of community nursing care under Commonwealth Home Support Package (CHSP) or Home Care Packages (HCP) • Be accredited against Aged Care Quality Standards • Be accredited against NDIS Practice Standards • Submit your application by the due date • Submit your application in full • Joint consortium applications are allowed if a lead entity is identified • Individuals cannot apply • Federal, State or local government agencies or bodies, and affiliate health organisations cannot apply • For-profit organisations cannot apply
To be eligible, the organisation must be a non-government organisation and a not-for-profit organisation. The funded activity must be delivered within the Newcastle and Lake Macquarie region. Applicants must be a current contracted provider of community nursing care under Commonwealth Home Support Package (CHSP) or Home Care Packages (HCP), and must be accredited against Aged Care Quality Standards and NDIS Practice Standards. Applications must be submitted by the due date and submitted in full. Joint consortium applications are permitted if a lead entity is identified.
