Who is eligible for the Chronic Conditions Prevention and Integrated Care Program?
- Must be an Aboriginal and/or Torres Strait Islander Corporation registered under the Corporations (Aboriginal and /or Torres Strait Islander) Act 2006, Company, Cooperative, Incorporated Association, Statutory Entity, or Partnership
- If applying as a Trustee on behalf of a Trust, the Trustee must have an eligible entity type
- Consortia are encouraged, but the lead organisation must be solely accountable to the Commonwealth and be an eligible entity
- Eligible organisations may partner with ineligible organisations in a consortium, except organisations listed by the National Redress Scheme as not joined or not intending to join the Scheme
- Applications may also be accepted from Non-Government Organisations, Primary Health Networks, Community Organisations, and Academic Institutions
