• Must have an approved Back to Work application for an employee who commenced employment from 2021-07-01 and is currently employed with the business. • Must be a small business employing fewer than 20 people. • Must explain how the funding will support retention of the Back to Work employee beyond 52 weeks of continuous employment. • Only one application is permitted per approved Back to Work employee. • Must provide quote or quotes totalling at least 2000. • Must declare they have not received, and are not eligible for, Australian Government funding for a similar purpose. • If applying for workplace fit-out, must provide evidence of ownership of the business premises or approval to update the premises. • Application must be submitted by the employer, not an agent or other party.
