- Provide a letter from a Qualified Agent supporting the revenue decline and employee count
- Not have received a $5000 grant under the 2025 Porepunkah Business Support Grant Program
- If a not-for-profit business, be registered with the Australian Charities and Not-for-profits Commission or a similar State or Territory regulator
